Home Care for Professionals
10 Ways UJC Makes Your Job Easier
Our team of case coordinators, registered nurses, social workers, and home health aides work together to make your job easier and your clients happier.
At UJC, we partner with discharge planners, MLTC agencies, CHHAs, geriatric case managers, medical professionals, and social service agencies to serve and keep clients. We help you find the right aide for your Medicare, Medicaid, and private pay clients.
A Large Pool of Qualified Aides
We have more than 800 fully certified Home Health Aides for short- or long-term care in all five boroughs and Nassau County for private pay, Medicaid or Medicare clients. All have had FBI background checks.
Reduced Direct Management Burden for You
We provide case coordinators and registered nurses who work directly with clients to resolve issues and connect clients to additional services, if needed.
Matches That Last
At UJC, we consider cultural and religious needs of clients as well as personal preferences and personality when assigning an aide. Our case coordinators know that compatibility matters in making a match. Whether it is knowing how to keep a kosher kitchen or speaking Mandarin, we find aides who make clients feel at home in their homes.
“I don’t normally work on Sundays but I did because a client needed me. Whatever time they call me—if they need me, I come.”
— Pamela, UJC Home Health Aide
Easy Referral Process
A phone call — (212) 460-5730 — is all it takes. Our case coordinators will do the rest.
With our large pool of aides, we can have an aide in the client’s home within 24 hours. We’ll follow up with a visit by a registered nurse and a more detailed intake interview to make sure the match is right.
Access to Social Work Services
Answers to questions about housing and accessing benefits are a phone call away. Families or clients may need counseling or help dealing with other problems. We will connect them with the help they need.
“If someone is identified as suffering from depression or confusion, that patient or family can reach out to a social worker and explore additional services.”
— Scott Garnier, Former Chief Operating Officer, UJC
Aides With Extra Training
Our aides are trained to recognize and deal with senior depression and dementia, and have the other skills needed to ensure that clients are cared for correctly, safely, and with the patience and respect they deserve.
Many of UJC’s Home Health Aides have been with us for decades. We pay a living wage and have created a culture of reliability and honesty.
No-Fee Help With Medicaid and Medicaid Pooled Income Trusts
Helping clients find the resources to get the help they need can be a challenge. The fees for assistance in accessing benefits can be exorbitant but not at UJC. We charge no fees above those mandated.
More Than 30 Years Experience
At UJC, we know how to navigate the benefit system and make it work for our clients. We step up when others walk away.
